well i'm not fimiliar with the older cars but here's my thoughts on your problems and a possible fix.
first some questions,
1. did you check the plugs (ALL OF THEM)?
2. do you have an egr? or did you delete it?
3. check pcv valve and make sure it is functioning correctly.
4. how warm is alittle?
possible fix, put a 180 degree thermostat in that bish to try and keep it cooler, then make a switch so you can turn on the fans manually so they run continuously. OR get the pcm tuned to turn on at 180 degrees.
those are some of my ideas, good luck!

Check your codes again... are you sure only 12 comes up?
This is from a OBDI code checking site... first disclaimer...
Note: code 12 is usually the first code you get and just ignore it.Watch the code after code 12, that's what you need.
I'm willing to bet though that your issues is your crank position sensor in the back of the block... Replace that and see what happens.

I also had a sneaky coil pack that worked great until it warmed up...

Yeah i had the same thing happen to a spark plug that was 2 months old with about 1000k on them. Ran great when cold but when warm it misfired bad.

Yea it only shows code 12 when the terminals are jumped. nothing else shows up.
I REALLY need to get this fixed. I hate not having a car....
